Advance Directive Planning

For anyone over the age of 18, completing advance directives is the most effective way to carry out your wishes when you are unable to speak for yourself. I am able to bridge the gap that exists between the hospital environment and the community by talking easily about life and death.  People think it’s not an easy topic to discuss, but it can be.  You get to decide what “living” really means to you and what you would consider a “good” death.  When discussions aren’t had or delayed, it leaves your family guessing about what you want or they feel enormous guilt that they are making the wrong decisions.  You don’t have to worry about putting your family through this stress and anxiety if you have it in writing and have asked someone to uphold your wishes.

Package includes:

  • A personalized review of Questions to Ask Yourself including your current and future health concerns
  • Designation of a Power of Attorney for Healthcare
  • Complete Advance Directives outlining your decisions regarding life-support, comfort care and after death planning
  • Up to 4 hours of personal consultation
